aee03ac03b Include catch header files in CMake script 
This doesn't matter for make systems, but for IDEs such as Visual Studio
on Windows, this allows you to edit Catch header files directly from the
project.

d43a47efca Refactored stream related stuff 
- simpler, polymorphic hierarchy-based, approach
- less bitty conditionals spread across the code
- all resolved up-front so now config class is immutable
(it had evolved the way it was and in need of a clean-up sweep for a long time)
